Job description

Job responsibilities

Co-ordinates the day-to-day provision of the CCN caseload, ensuring patients needs are met, new referrals triaged in a timely manner and workload allocated according to need.

To undertake clinical assessments, implement, deliver, and evaluate nursing care for children and young people within a defined caseload in clinics, patients homes, schools or other appropriate community settings.

To deliver skilled clinical nursing care which may include administration of Intravenous therapy, injections, syringe pumps, blood taking via central venous access devices, wound care, nursing support for all types of enteral feeding tubes, tracheostomy care, bowel and bladder devices.

Using a case-co-ordination approach support families to understand and manage their childs condition safely at home and in collaboration with multi-agency providers to ensure the childs aspirations and health needs are central to decision making and maximise their opportunitiesto develop.

To provide patient/parent/carer training and assessment of competence.

To assist in the discharge planning process of a childs transfer from hospital to home to identify clinical care and support that can be safely delivered in the community.

To act as a signpost for families enabling them to access support from a range of statutory and voluntary organisations.

If you have a non-medical prescribing qualification, you will be expected to practice as a prescriber as part of this role.

To ensure that clinical guidelines and electronic patient record systems are evidence based and maintain clinical effectiveness.

Person Specification

Qualifications

Essential


  • RSCN or RN Child/Learning disability with NMC registration
  • MSPP or other relevant teaching/assessment qualification.
  • CAKES - or equivalent Children's Assessment Knowledge Evaluation and Skills course

Desirable


  • Non Medical Prescribing (V300)Course
  • If not already completed then there needs to be a willingness to undertake Specialist Practitioner Qualification (Children's Community Nursing)

Experience

Essential


  • Experience of caring for children with short term acute and long term conditions
  • Experience of working under pressure to prioritise the needs of a clinical area
  • Experience of allocating workload and supporting staff to maintain patient safety.
  • Demonstrates an understanding of Community Nursing

Desirable


  • Experience of day to day supervision and/or line management of staff
  • Experience of nursing children with long term conditions

Skills and knowledge

Essential


  • Experience in delivering a range of clinical nursing care for example:- oAdministration of Intravenous medicines via a range of devices. oCare of enteral feeding tubes oWound care
  • Evidence of teaching others and assessing clinical competence of parents, non -registered staff and carers
  • Ability to develop effective working relationships with the child and family
  • Ability to work in a multi-agency team
