As a key member of our Continence Service, you will play a vital role in providing expert care and support to children and young people with bladder and bowel dysfunction.

Job Summary:

The successful candidate will be responsible for assessing the needs of individuals in the community and school settings, providing individualized support to children, young people, and their families, and promoting continence and guiding toileting. You will work closely with our Continence Specialist Nurses to deliver community care and develop training programs for staff, inclusive of education, social services, and health.

Main Duties:
  • Provide individualized evidence-based clinical assessment, review, and advice to optimize continence for children and young people with/without Special Educational Needs and Disability.
  • Support toileting workshops, devise and supply individualized toileting advice and care plans.
  • Give clinical advice and support to parents, carers, and education staff to help them understand childhood continence conditions and support their child/young person with advised treatments.
  • Be responsible for ensuring timely continence assessment and completion of reports/information requests to support Child in Need/Child Protection meetings and Education, Health, and Care Plan (EHCP) procedures.
  • Facilitate/participate in the program of education to support the promotion of early identification, treatment, and management of childhood continence issues.
  • Plan and organize own work, including triaging referrals, clinics, administration, and meetings, prioritizing work accordingly.
  • Ensure that referrals to other health and social care partners are completed in a timely manner in conjunction with colleagues in the Continence Service and, where appropriate, the multiprofessional team.
  • Assess the need for containment products, following local and national guidance, and have an awareness of the budgetary costs associated with this.
  • Contribute to transition planning with adult services.
Communication:
  • Ability to communicate with the Child/Young person of all ages and their families/carers, utilizing a range of communication techniques and styles to promote continence.
  • Work in partnership with Child/Young person and carers to enable them to make informed choices about care and treatment to promote their physical and mental health well-being.
  • Complete record keeping, using an electronic patient record system, in line with Trust policy and guidelines supporting effective record keeping.
  • Record incidents in line with Trust guidance and support with learning from incidents within the service and across the wider Children and Specialist Nursing Teams.
  • Ability to communicate with professionals both internal and external to the Trust to understand and consider how they might support child/young person's agreed continence care plan for a positive outcome.
  • Network and maintain effective working relationships with multi-disciplinary/multi-agency professionals across primary and secondary care to ensure appropriate/individualized positive continence outcomes are achieved.
  • Develop and deliver training to Children's Services staff in education, health, social care, and other identified partners as required.
  • Recognize barriers and utilize a range of resources to empower children, young people, their families, carers, and other professionals to promote continence.
  • Provide clear, non-ambiguous written nursing reports and management plans with the child, young person, their families, and other carers to support Child in Need, Education, and Health Care Plans and other statutory requirements.
Budgetary Responsibility:
  • Assess the need for containment products, following local and national guidance, and have an awareness of the budgetary costs associated with this (attending meetings with containment products suppliers and other relevant meetings).
  • Maintain stock and equipment required for the children's continence service clinics to be safe and effective within budget and ensuring best value for money.
People Management:
  • Acting as a resource for staff supporting the continence caseload (registered and non-registered staff in the community nursing and specialist nursing team) example of supporting staff within the special school nursing team with implementation of toileting plans.
  • To be responsible for facilitating supervision for continence champions in 0-19 and special school services.
  • Maintaining own professional development including clinical supervision in this specialist area and requirement to take part in appraisal.

Qualifications and Training:

  • Registered Qualification - Nursing Degree
  • Current professional registration with NMC
  • Teaching/Practice Supervisor or Practice Assessor experience

Desirable:

  • Specialist Community Qualification
  • Member of relevant professional continence forums
  • Completion of children's specialist continence training courses
  • Completion of SEND specific training
  • Clinical assessment and examination skills

Experience:

  • Experience of managing a caseload with demonstrable evidence of supporting children and young people with SEND needs
  • Knowledge of children's continence issues
  • Demonstrable experience of working with children and young people with EHCP needs, participating and completing documentation/supporting professional meetings
  • Experience of working autonomously in clinical practice
  • Evidence of planning and delivering training to a wide range of professionals to help understanding and support of managing a health condition

Skills:

  • Excellent communication skills
  • Ability to provide Specialist clinical advice and guidance both within and across agencies.
  • Literate in IT and Computer Skills and ability to communicate through IT using packages such as Word, Outlook, Excel, and PowerPoint
  • Report writing skills
  • Ability to work as a team and as an autonomous practitioner
