ULHT's Frailty Service have an exciting opportunity for a Frailty Advanced Clinical Practitioner - Band 8a

Main duties of the job

Successful candidates must be a qualified ACP not necessarily within the specialism of Frailty as full support will be given to complete Frailty Competencies. Be flexible, innovative, highly motivated and above all be committed to improving the care and assessment of frail older people in Lincolnshire.

Location: Pilgrim Hospital. The post holder will be identified this as their base site, but will be expected to work across all sites within ULHT to meet the needs of the service. There may also be a requirement to work as needed across partner organisations as part of the vision for an Integrated Frailty Workforce for Lincolnshire.- Permanent position- The service will eventually run 7 days a week . Hours 8.00 - 20.00

This is a fantastic opportunity to be a part of our dynamically evolving Integrated Frailty Service. Successful candidates will work as part of our Front Door Service providing clinical expertise in A& E, our Emergency Admissions Units and in our Frailty SDEC. Successful candidates will also input into the county wide Frailty Virtual Ward. Successful candidates with strive to prevent unnecessary A& E attendances, hospital admissions and promote care closer to home principles.

Job responsibilities

The Frailty ACP will use specialist knowledge and skills to provide autonomous assessments to patients. The ACP will be accountable for independent clinical assessment, diagnosis and treatment of patients who are frail as part of the Front Door Frailty Service.The aim is to provide prompt specialist assessment to avoid hospital admissions where appropriate, to deliver a SDEC frailty model of practice and to promote care closer to home through our Frailty Virtual Ward

Qualifications

Essential


  • Current professional registration within area of practice
  • First degree
  • Post registration qualification or training relevant to speciality
  • MSc in Advanced Clinical Practice
  • Registered Independent prescriber
  • Evidence of teaching, supervision and assessment

Desirable


  • MSc in Frailty
  • Older adults fellowship
  • Teaching Qualification

Previous Experience

  • Substantial post reg relevant experience

  • Management and Leadership experience
  • Completed portfolio of supervised practice
