The Berkshire Early Intervention in Psychosis Service (EIP) is looking for a passionate and enthusiastic band 6 mental health practitioner to join their innovative and dynamic team. We are a specialist service working with individuals experiencing a first episode of psychosis or who are presenting with an at-risk mental state, between the ages of 18 to 65. We operate a Monday to Friday 9am to 5pm service, although some flexibility is required. You will have a clear passion for supporting people and their family in their recovery. To act as care coordinator, in accordance with the Care Programme Approach (CPA) policy under the supervision of the Clinical Lead. The role is focused on working collaboratively with the client and their family/friends on their recovery, developing a relapse management plan.
To undertake comprehensive assessment of health and social needs together with the client and their family to implement a plan of care and provision of NICE recommended interventions.
To work closely with other stakeholders to ensure the client has a comprehensive and holistic care plan to meet their well being needs.
To monitor and assess the physical health of service users in line with Trust policy and national guidelines.
Plan and provide treatment in collaboration with a multi disciplinary team and ensure clients are on the correct care pathway

Diploma or degree level professional qualification, and registered for practice through NMC or HCPC
Experience or being a care coordinator.

Further relevant qualification in the field of mental health - CBT /PSI or other


Experience of working in community mental health services
Minimum two years post-qualification experience of working with adults with mental health problems.
Experience of assessment and report writing.

Thorough working knowledge of mental health problems and their management.
Knowledge of current mental health policy
Ability to carry out high quality assessments and treatment interventions to individuals and groups
Ability to maintain good quality records, and write reports
Ability to reflect and critically appraise own performance
Basic computer literacy and ability to use electronic system

Ability to travel to multiple sites
