The Bordon and Petersfield Community Mental Health Team operates from Petersfield hospital where this is our main site. The CMHT provides services to patients in the Bordon and Havant area as well . The Community Mental Health Team is focussed on delivering a recovery service for clients with severe and enduring mental health issues. We support our staff in developing their clinical skills in attending post graduate training such as physical health and nurse prescribing.

Main duties of the job


  • To provide recovery based community mental health services within a multidisciplinary team to adult individuals with severe and enduring mental health needs referred to the Community Mental Health Team.
  • This role, in collaboration with referred individuals and where appropriate family or carers, will coordinate and provide recovery focussed assessments, risk formulations, care planning, crisis and contingency planning, evidence based interventions, response, reviews, reports, discharge planning and attendance at appropriate meetings.
  • Role further includes acting as clinical and case management supervisor and mentor.
  • To make links with the Primary Health Care teams.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
