We are looking to appoint to Specialist Doctor in Geriatric Medicine to at Sheffield Teaching Hospitals NHS Foundation Trust.
The successful candidate will be primarily based at the Northern General Hospital, but may be required to work at both campuses in the Trust.
A medical degree or diploma from a recognised UK or overseas university or institution. Full registration with the General Medical Council with a licence to practise. A minimum of 12 years medical work full time since obtaining a primary medical qualification (or equivalent gained on a part-time or flexible basis) at least 6 of which will be in a relevant specialty working at Specialty Doctor and/or closed SAS grades. Equivalent years' experience with evidence of relevant competencies achieved in a related specialty from other medical grades including overseas will also be accepted.
Any candidate who is unable for personal reasons to work full-time will be eligible to be considered for the post if such a person is appointed, modification of the job content will be discussed on a personal basis with the Trust in consultation with consultant colleagues.
The successful candidate will assist the directorate in developing our liaison services, and a liaison geriatrics session will be job planned. The specifics of this session can be tailored to individuals' experience and interest. - Medical Degree from a recognised UK or overseas university or institution
- A minimum of 12 years full time Medical work (or equivalent gained on a part-time basis) since obtaining a primary medical qualification at least 6 of which will be in a relevant specialty at Specialty Doctor and/or closed SAS grades or equivalent
- Full registration with the General Medical Council with a licence to practice
- MD or PhD or equivalent
- MRCP or equivalent
- Manages the difficulties of dealing with complexity and uncertainty in the care of patients, employing expertise and clinical decision-making skills of a senior and independent/ autonomous practitioner
- Manages complex common presentations in older people, including falls, delirium, dementia, movement disorders, incontinence, immobility, tissue viability, and stroke
- Performs comprehensive assessment of an older person, including mood and cognition, gait, nutrition in an inpatient setting
- Manages a multi-disciplinary team including effective discharge planning
- Recognises patients approaching the end of life, initiates timely advance care planning conversations, and leads the provision of high quality palliative care with skilled and sensitive communication.
- Demonstrates the underpinning subject specific competencies to enable practising at senior grade as evidenced in job plan, portfolio and logbooks
- Clinically evaluates and manages a patient formulating a prioritised differential diagnosis initiating an appropriate management plan with the ability to review and adjust this depending on outcomes of treatment
- Critically reflects on own competence, understands own limits, and asks for help when required
- Wider experience in different units in related specialties
- Demonstrate an understanding of management & Clinical governance issues
- Collaborate and respecting the multidisciplinary and interprofessional teams to manage risk issues across organisations and settings
- Understands the principles outlined in the GMC document Good Medical Practice
- Understands the requirements of Medical Appraisal and Revalidation
- Evidence of participation in CME/CPD in the last 12 months
- Maintenance of a CPD diary or portfolio
- An understanding of the issues of clinical effectiveness and audit
- Ability to critically assess published research and incorporate it into clinical practice where appropriate
- Documented completed audits during training/Experience
- Understanding of the issues surrounding clinical research
- Experience of teaching student doctors, doctors in training and/or other allied professionals at all levels.
- Good written communication skills
