Brent Integrated Care Partnership has a number of exciting opportunities available, supporting the borough based partnership to deliver its objectives in a changing and evolving health and care landscape. Main duties of the job

This role presents a unique and exciting opportunity to spearhead a substantial transformation initiative, uniting the National Health Service (NHS), Council, and Voluntary, Community, and Social Enterprise (VCSE) organisations in Brent.

The primary aim is to bring essential services closer to the community, fostering an environment that enhances care, health, and overall well-being while mitigating health disparities throughout the Borough.

The overarching vision is to elevate the quality of care, health, and well-being for Brent's local residents, ensuring service provision aligns with the community's needs, maintains cost-effectiveness, and produces optimal outcomes for service users. Central to the success of this vision are robust integrated neighbourhood teams and relationships, underpinned by a coordinated strategy across Brent ICP and the broader Northwest London Integrated Care System (ICS).

Collaboration with local partners in the NHS, Brent Council, and VCSEs is paramount, with an emphasis on commissioners and providers working collaboratively as a unified partnership system, placing patients and residents at the core of decision-making.

The ICP is dedicated to improving the health and wellbeing of residents and is committed to working with all of our diverse communities to ensure everyone's needs are met.

We aim to place the resident at the heart of health and care services in order to tackle health inequalities together - working as equal partners and with a collective goal to improve the wellbeing of everyone in Brent.

Because we want the roles to be available to people from a range of different professional backgrounds, these roles are being advertised in CNWL and at Brent Council. The roles will be based at Brent Civic Centre in Wembley, and will involve 3 days working in the office per week, with home working available for the other days.

MAIN DUTIES AND RESPONSIBILITIES

Strategic Leadership

Lead the detailed mapping of services to geographical neighbourhood footprints across the Borough

Develop and implement a neighbourhood model based on national best practices, securing buy-in from system partners to the overall approach

Identify integrated health and care hub sites, working with partners to bring services and staff together, i.e. co-location of services

Work with system partners to develop detailed plans for bringing services and staff together

Working within the overall strategic objectives, devise, implement and monitor the strategy. Evaluate, interpret and locally implement best practice.

Provide overall management to the function.

Develop and implement qualitative and quantitative measures to determine performance against the organisational strategy. Report progress against the strategy through personal representation at senior management forums and by written reports to appropriate boards and groups of staff.

Present complex information on all aspects of function practice in a clear, understandable and audience-appropriate manner to senior management and board level groups.

Ensure that best practice is developed and delivered at organisational and departmental levels. Challenge ways of working and persuade, motivate and influence other senior managers to realign their practice where necessary.

Ensure a smooth transition through decision making process to the management of contracts.

Ensure that an appropriate strategy is in line with business objective and are fully cascaded.

Personally lead, support and contribute to formal negotiations with senior level staff from external stakeholders, providing a high level of negotiating expertise to secure the most advantageous arrangements.

Advise on innovative opportunities and support all departments in their strategies and programmes to maximise service benefits.

Oversee the tracking of progress against plans and transition milestones, ensuring appropriate processes are in place to flag issues, risks and concerns with the relevant stakeholders.

Work closely with the Heads of other Department or Directorate to ensure that units understand and adhere to rules and standards, intervening as appropriate.

Ensure the securing of value for money, giving due consideration to all relevant factors including risk, quality and other factors.

Person Specification

Education and Qualifications

Essential

  • Relevant professional qualification at a master's level or equivalent.
  • Programme / Project management qualification, or equivalent experience in programme delivery roles

Desirable

  • Evidence of Continuing Professional Development
  • Formal Leadership Qualification

Experience

Essential

  • Significant experience in leading large-scale transformation programmes within health and social care settings.
  • Proven experience in stakeholder, largely community engagement and co-production

Desirable

  • Extensive, in-depth knowledge and experience across a range of relevant areas, including: finance, contracting, people management and programme management.
