Respiratory Specialist Practitioner - Hospital@Home (NHS AfC: Band 6) - Nursing and Midwifery - Somerset NHS Foundation Trust
Somerset NHS Foundation Trust – Yeovil, Somerset
Hospital @ Home - Respiratory Specialist Practitioner - Band 6
This is a community post based across our Yeovil Neighbourhood therefore you must be a car driver with UK driving license and access to your own vehicle (we'll pay mileage)
A vacancy has arisen, creating an exciting opportunity to join the respiratory element of the Somerset Hospital@Home programme.
H@H is designed to facilitate early/supported discharge of patients from acute medical beds and to prevent hospital admissions of patients with unstable respiratory disease in the community, so creating a ‘virtual ward’. A mix of Band 3/4/6/7/8 staff will provide the respiratory service, supported by senior medical professionals, and work across the four Somerset neighbourhoods.
We are a friendly, supportive team and there are opportunities for development within this post through completing relevant courses such as PACR. The respiratory team will align closely with colleagues in other evolving community services (notably the frailty H@H virtual ward team). This is a 7 day a week service.
Respiratory Specialist Practitioners work as part of a multi-disciplinary clinical team across a wide range of settings, including hospitals, community clinics, individual’s homes, care homes and general practices. RSPs work at an enhanced level of practice with specific knowledge and skills in a field of expertise which, for these posts, is respiratory medicine. They manage a discrete aspect of a patient’s care within their current level of practice, which will be particular to a specific context.
As an organisation, the NHS offers a wide range of benefits including flexible working, pension contributions, market leading annual leave allowance, career progression and regular conversations, not to mention our Blue Light Card and NHS exclusive discounts.
The benefits of working in Somerset include the idyllic countryside, with our areas of outstanding beauty and stunning coastlines. You will get to enjoy these perks whilst still only being a stone’s throw away from bustling city centres like Bristol, Bath and Exeter and only two hours away from London.
There are excellent educational facilities in the area and, when compared to other regions, house prices are reasonable. You will experience the best of both in Somerset, the countryside and the cosmopolitan – there is truly something for everyone
The broad purpose of the role is to provide a high standard of complex, enhanced care for patients, using enhanced levels of clinical judgement, skills and knowledge. Don't transfer any money or complete suspicious online surveys.
