The Community ACP is an integral part of the Integrated Care Team and works collaboratively with the wider team to provide clinical expertise and support for managing people with complex medical and social needs, acute illness, and management of Virtual Ward Patients.

The post holder will utilise their advanced clinical skills to reduce unnecessary non elective secondary care usage and provide comprehensive, advanced assessment, treatment, care, support and advice to patients and families.

The B7 Community ACP will hold the Post Graduate Diploma in Advanced Practice (APACs and non-medical prescribing).

Main duties of the job

The Community ACP will provide a holistic approach to prevent unnecessary hospital admissions and facilitate early discharge from secondary and Intermediate Care. This will include first diagnosis of undifferentiated conditions, instigating new treatment plans and regimes based on evidence based practice, independent prescribing and follow up.

An essential part of the Community ACP role is the ability to work autonomously, using own initiative and clinical reasoning in a variety of care settings. Whilst being able to demonstrate the ability to prioritise workload, provide support and education to the wider MDT.

The Community ACP will work within the Integrated Care Teams and Virtual Ward as a resource to provide clinical information, to support the team with case management, care planning, interventions and monitoring of patients with complex needs including patients with multiple complex conditions. Where clinically appropriate the Community ACP will be the first point of contact for the Community Nursing teams where assessment, diagnosis and treatment is required. The ACP will also work closely with the IGSM MDT within the Virtual Ward. The Community ACP will also assess Yorkshire Ambulance Service Category 3/4 calls when required.

The successful candidate would be expected to be flexible and provide cover where needed city wide / dependant on service provision, covering hours between 8am - 8pm over a 7 day period.

Education and Qualifications

Essential


  • Registered Nurse with Valid NMC registration or Health Care Professional and registered with HCPC with relevant clinical experience
  • Willing and able to successfully complete a full Masters in Advanced Clinical Practice - which may be via the apprenticeship route
  • Post Graduate qualification in Advanced Practice
  • Advanced Physical Assessment module, Non-Medical Prescribing
  • Advanced life support qualification (ALS/ILS) or willingness to undertake
  • Evidence of relevant continuous professional development

Desirable


  • Recognised Teaching & Assessment Qualification or equivalent experience
  • Recognised Leadership Award/qualification or equivalent experience

  • In depth knowledge of pathophysiology of common conditions seen in the speciality and their management
  • Able to analyse and synthesise patient information to formulate appropriate diagnoses and management plans

  • Can prioritise and manage own workload and support junior personnel in managing theirs when faced with conflicting pressures
  • Ability to manage self and maintain professional conduct in difficult and challenging situations
