An exciting opportunity has arisen to join an established team of Community Children's Nurses in Birmingham on a permanent basis. This is a city wide service that offers specialist nursing support to children & families with a variety of complex physical health needs.


We provide clinical interventions to children within their own homes, schools and other community locations. The service operates seven days a week and also provides an out of hours 'on-call' service.


If you are an experienced Children's Nurse with excellent clinical skills, have advanced communication abilities you may be who we are looking for to join our busy team. This post will provide the successful candidate with the opportunity to develop rewarding therapeutic relationships with children & families in a community setting.
You will be required to work in conjunction with the Children's Rapid Response Service whose aim is to prevent a hospital admission and the Children's Palliative Care Team insupporting parents, carers and children with life limiting conditions and in the terminal phase.


The post holder will be responsible for a defined caseload of children delivering high quality nursing care according to agreed standards which are set and monitored.

Work in accordance with NMC Code of Professional Practice and Scope of Professional Practice. Provide comprehensive evidenced based and skilled nursing care to children within their own home and/or other community settings.


~ Ensure assessment, planning and delivery of holistic care is of the highest standard utilising an agreed nursing model.


~ Work in partnership with children, families and carers to provide optimum individual outcome acknowledging children and familys personal preferences and lifestyles.


~ Liaise and work in collaboration with acute services, primary healthcare team and other agencies when planning and implementing programmes of care.


~ Provide the clinical lead in care packages funded by the Community Complex Care team when allocated to the Community Childrens Nursing (CCN) caseload.


~ Provide health education information to the child/young person, family and carers to promote a healthy lifestyle through health promotion activities as required.


~ Plan and participate in teaching packages to families and carers to ensure evidenced based care is delivered.


~ Advise the Team Leader, Clinical Lead or Service Manager of any circumstances which give cause for concern in relation to patient care and/or any concerns raised by the primary healthcare team or partner agencies.


~ Undertake home visits to children registered with GPs within Birmingham as required.


~ Undertake home visit risk assessments to ensure safety during visits especially those during unsociable hours.


~ Obtain informed consent from Children, Parents and Carers prior to undertaking care.


~ Maintain clear, concise and contemporaneous records adhering to BCHC record keeping policies.


~ To be professionally and legally accountable for nursing practice and all activities.


~ Provide a flexible service to accommodate the working environment.


Qualifications / Training
RSCN/RN Child Branch
Community Children's Nursing qualification or equivalent

Children's Palliative Care experience
Significant experience of working as a Children's Nurse either RSCN/RN Child


Experience of working with children with life limiting/ threatening illnesses and end of life


Demonstrate knowledge relating to child protection issues and procedures
Evidence of assessment skills and use of nursing process and nurse models
Demonstrate sound use and application of NMC Code of Conduct and scope of professional care
Demonstrate knowledge of current legislative changes affecting NHS and Social Services particularly those affecting community care
Demonstrate knowledge and application of current clinical trends and practices inclusive of evidence based practice in relation to Children's Community Nursing ability to teach relatives, patients, clients and staff across all areas of self care and appropriate nursing practice
Demonstrate individual responsibility in respect to health and safety
Awareness of diversity of the needs of families in relation to culture and religion
Personal Qualities
Demonstrate ability to be flexible and participate in an NHS change environment
Positive attitude to service development and Children's Community Nursing and Palliative Care

Ability to work flexible hours, evenings, weekends, Bank Holidays
