Job Requirements:

At least 2 years of post-qualifying experience as a qualified social worker
Good communication, time management, and IT skills
Knowledge of The Care Act and Mental Capacity Act

Responsibilities:

Work in a multi-disciplinary team and in partnership with other agencies
Undertake Adult Safeguarding investigations
Take an active role in Decision Support Tool Meetings for joint health-funded packages
