Our Discharge to Assess (D2A) team plays a crucial role in facilitating smooth transitions from hospital to home or other care settings, ensuring individuals receive the appropriate assessments and support to continue their recovery and maintain their independence.

Role Overview: We are seeking a dedicated and qualified Social Worker to join our Discharge to Assess team. You will have a strong background in conducting assessments under the Care Act, as well as experience in completing Mental Capacity Assessments and Safeguarding Enquiries. Knowledge of the Discharge to Assess model is highly desirable.

Key Responsibilities:

Conduct comprehensive assessments under the Care Act to determine individuals' needs and eligibility for services. Perform Mental Capacity Assessments to ensure individuals' decisions are respected and supported. Undertake Safeguarding Enquiries to protect vulnerable adults from abuse and neglect. Work collaboratively with multidisciplinary teams, including healthcare professionals, to facilitate timely and safe discharges from hospital. Develop and implement personalized care plans that promote independence and well-being. Provide expert advice and support to individuals, families, and carers throughout the discharge process. Maintain accurate and up-to-date records in accordance with legal and organizational standards. Stay informed about relevant legislation, policies, and best practices in social work and discharge planning.

Qualifications and Experience:

A recognized qualification in Social Work (Degree or equivalent). Registration with Social Work England. Proven experience in conducting assessments under the Care Act. Demonstrated expertise in completing Mental Capacity Assessments. Experience in undertaking Safeguarding Enquiries. Knowledge of the Discharge to Assess model is preferable. Excellent communication and interpersonal skills. Strong organizational and time-management abilities. Ability to work effectively both independently and as part of a team. Commitment to continuous professional development and reflective practice.
