Mental Health Outreach WorkerMonday - Friday £25,584 per annum Brook Street Social Care are working with a reputable mental health charity in Cardiff to join a close-knit well-established team. If you are passionate about making a real difference in people's lives - then apply today The role will involve supporting adults with mental health needs in the community and their own homes.Key Responsibilities:Provide one-to-one support for adults with mental health conditions within their own homes and local communities.Manage a caseload of vulnerable adults, conducting assessments and reviews of their mental health and wellbeing.Assess information from a variety of sources, ensuring the appropriate services and support are identified and delivered.Facilitate service user involvement, ensuring their voice is central to all aspects of their care and support.Address housing management and organisational issues that may impact service users' mental health or wellbeing.Work collaboratively within a supportive and dedicated team to deliver high-quality care and support services.Requirements:A valid driving license and access to your own vehicle, with business insurance.Experience in supporting adults with mental health conditions is preferred, but we will consider applicants with a strong commitment to making a difference.Strong communication and organisational skills, with the ability to assess and review complex information.Ability to work independently and as part of a team, with a proactive approach to problem-solving.
