You will provide the central community-facing access point for adult social care enquiries and safeguarding concerns.
Responsibilities- Manage a varied caseload working with professionals across the team to ensure a seamless service to individuals with a range of needs.
- Carry out a range of social care tasks including screening and triage, initial assessment, support planning and case coordination to an exceptional standard.
- Support people to identify their strengths and networks, delaying and preventing the need for new or additional social care intervention wherever possible to enable people to continue living life in the way that they wish.
- Exemplify best practice in working with adults in strengths-based and person-centred way, connecting people with informal, community-based support wherever possible.
- Be fully committed to working in partnership with residents, carers, community-based organisations and other key professionals involved in enabling people to be as independent as they possibly can.
