Are you ready for a new and exciting challenge, working as an  Independence and Wellbeing Practitioner  in a role that is an integral part of the Hospital to Home Team or the Flexible care and support teams delivering a high-quality service to the people of Waveney?

Post 1: James Paget Hospital, Gorleston, NR31 6LA / Onsite. Posts 2 and 3: Riverside, Lowestoft, NR33 0EQ and Carlton Court Hospital, Carlton Colville, NR33 8AG / Hybrid.

Your role and responsibilities

Waveney Area in Suffolk County Council is aligned to the Great Yarmouth and North Suffolk Integrated Care Board (ICB) and we are committed to working towards an integrated health and social care system to support better outcomes for adults in our area. You will play a key part in supporting this work to ensure effective and efficient discharges for patients, assessing their needs to determine the best outcomes. You will be based within the Hospital to Home Team at James Paget University Hospital or the Flexible Care and Support Team at Riverside. 

Post 1:

You will be based at the James Paget University Hospital in Gorlestonin the Discharge Planning Hub alongside both health and social care colleagues. There is an expectation that you will work across all discharge pathways making visits to the ward, liaising with ward staff, other stakeholders, and community resources.

You will use the Signs of Safety model to support assessment of the patient to ascertain their wishes and feelings and support them to achieve greater independence following their discharge from hospital.

Posts 2 and 3: 

You will be based within the Flexible Care and Support Team in Lowestoft and also be working from Carlton Court Hospital in Carlton Team's remit is to provide support across the area as needed, with a special focus on reviewing support plans, provider failure work, mental health hospital discharges, scheduled and unscheduled reviews. This role provides a variety of case-work in a supportive environment.
