As a Senior Practitioner you will be an integral part of supporting the Hospital to Home Team delivery of high-quality services to the people of Waveney. 

Your role and responsibilities  

In this pivotal role, you will collaborate closely with the Team Manager, spearheading effective hospital discharge processes, fostering strong partnerships with healthcare providers, and identifying opportunities for optimised service delivery and cost reduction.

Supporting the Team Manager in the Leadership and management of the Hospital to Home team, ensuring effective operations in planning, and staff well-being and development. You will play a vital part in monitoring service performance, promoting best practices, and escalating risks appropriately. Your robust budgetary management skills will ensure spending targets are met while delivering cost savings. Additionally, you'll drive staff engagement with systems transformation, working hand-in-hand with the transformation team to implement and embed cutting-edge information, technology, and digital solutions.

You will need  

experience as a Senior Practitioner to support the challenges of this role we hope you will bring practice experience and an understanding of integrated working  our ideal candidate will offer a supportive approach and positive leadership skills  experience of supervising staff, leading peer learning, and supporting staff with complex casework some knowledge of hospital discharge and reablement.

The team

Waveney is aligned with the Great Yarmouth and North Suffolk Integrated Care Board (ICB) and is committed to working towards an integrated health and social care system to support better outcomes for adults in our area. You will play a key part in supporting this work alongside the Hospital to Home Team Manager, Senior Practitioner, and a dynamic hardworking team of Social Workers, Independence and Wellbeing Practitioners and Occupational Therapist. The team sits in both Riverside and James Paget University Hospital discharge hub supporting timely hospital discharge, monitoring of reablement beds and assessment of long-term care in the community.
