Clinical/Professional Duties

**The role of the Care Home Practitioner will include**:

- In discussion with the registered practitioners of the integrated team, patient and carers, support the development of an individual plan of care through assessment of the immediate and ongoing needs of the patient. This should be in accordance with Trust policies, procedures, protocols and guidelines.
- Contribute to the development of health promotion and education for patients and carers within agreed guidelines.
- Put into practice the care plan written by the registered practitioner by undertaking care activities.
- Report to the registered practitioner any changes in the patients’ health and wellbeing.
- Document any changes in the patients’ notes.
- Work in partnership with all members of the integrated team both internally and externally to the Trust to ensure continuity of care for the patient. Attend MDT meetings and care home meetings when required.
- Deliver clinical/technical interventions and/or therapeutic/rehabilitative care to patients based on the individual needs identified under the guidance of the relevant professional where required in accordance with the care plan.

MAIN DUTIES AND RESPONSIBILITIES

- Support the assessment and management of patient care needs through monitoring of progress, feedback discussions, reports and multi-disciplinary meetings.
- Work with and alongside care home staff, relatives and carers, sharing knowledge and skills to promote understanding of the aims of intermediate care and/or rehabilitation to ensure a consistent approach to care provision.
- Perform extended skills following training and assessment of competency with registered practitioner.

Patient/Client Care
- Undertake reviews of patient healthcare needs as required against the plan of care, taking appropriate action to achieve the planned aims as directed by the relevant professional.

Communication Skills
- Establish a therapeutic relationship with the patient, carers and relatives and care home staff to engage them in health promotion, prevention and rehabilitation processes.
- Adapt communication style to meet the needs of the individual to breakdown any existing barriers to communication. Using appropriate methods to convey messages and support the expression
