As an Integrated Therapy Coordinator working within the Joint Community Rehabilitation team, you can make a real difference to a person’s life.

The role will include working in partnership with therapy and support staff to establish peoples’ needs and identify goals for them to work towards. 

The role will oversee the provision of daily living equipment to help people maximise their potential and promote their independence in their home.

We are looking for candidates who:

can use their organisational skills to coordinate the deployment of therapy staff. will ensure that all relevant assessments are completed in a timely manner can assess and reduce risks to allow clients to remain as safe as possible in their own home, using a reablement approach. are able to work collaboratively with professionals within our service and from external/partner organisations have communication skills are able to work effectively with a range of professionals to achieve the best outcome for out clients

We will support you to develop the skills you’ll need to manage the challenges of this role which can be fast paced and require you to work alongside, and negotiate with partners.
