Purpose Of The Post:

  • Ensure that the people of Gwynedd are at the heart of everything we do.
  • Assist individuals with learning disabilities to live their lives in the way they wish to live, assist them to develop skills and ensure opportunities to be a part of their community.

Responsibility For Functions:

  • Ensure appropriate use of specialist equipment including reporting on deficiencies and following practice protocols and specialist training as required.
  • Ensure appropriate use of users' property e.g. medicine and cash.

Main Duties:

  • Support The Individual Who Use The Service
  • Work to meet the well-being objectives of the individuals you are supporting and ensure and respect individuals' rights and empower their voice.
  • Assist with personal care in a dignified and respected manner.
  • Contribute to and identify opportunities to amend and follow the individuals Personal Care Plans and encourage taking positive and well-informed risks.
  • Contribute to the assessing, monitoring, planning and implementing process according to the needs of the individual.
  • Test new ways of supporting and use creative solutions that draw on strengths.
  • Communication And Relationship
  • Co-operate and communicate with professional workers and families/carers and search for solutions that can help. Contribute to multi-disciplinary discussions.
