Full job description

We are looking to recruit enthusiastic, passionate and motivated community practitioner to join our Perinatal Mental Health Team covering the East Staffs area. We are a specialist service that strives to deliver excellent standards of care to expectant and new mothers with severe and complex mental health problems.

This is an exciting opportunity to be part of the perinatal service and to help shape the future of perinatal mental health care in Staffordshire. You will be joining a friendly and supportive multidisciplinary team with extensive training opportunities along with regular supervision, reflective practice and opportunity for career progression.

As a member of the team you would provide specialist assessments and interventions to women and families during pregnancy, delivery and up to 2 months postpartum. You will continue to develop upon our strong links with Midwifery, Health Visiting, Maternal Mental Health Service, Family Nurse Partnership, Adult Mental Health, IAPT, CAMHS services and voluntary sectors.

We have adapted to new ways of working due to pandemic, and some of the blended working developments will continue as clinically indicated. The team works closely with the Brockington Mother and Baby Unit in Stafford, and with the Shropshire Community Perinatal Mental Health Team.

This post will cover Cannock and the South Staffs area.

Take responsibility for the management of a complex caseload, which includes, specialist assessment, care planning in conjunction with service user and carer, evaluation of individual treatment plans.

Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.

Assess patient's individual care needs, develop, implement and evaluate programmes of care.

Deliver a range of interventions in accordance with the agreed plan of care and pathway guidance including; pharmacological and psychologically informed interventions.

Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.

Maintain an effective reporting system by observing and reporting verbally and in writing on patient condition

Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.

Maintain an up-to-date level of professional competence within the specific environment.

Assist in the implementation of evidence-based practice and participate in research and other projects as required.
