Social Worker, Young Adults Transitions Team, Bristol
Pertemps are recruiting for a qualified and experienced Social Worker to join the Transitions team. The postholder will lead the way to ensure the best possible outcomes for young people aged 14-25 years in the service. They will support completion of Care Act assessments and support plans leading to the commissioning of services to meet identified need where non-commissioned support, Technology, equipment, or wider community assets could not ensure independence outcomes.

Pay rate – competitive

The successful candidate must have prior experience within adult social care, with experience in legislation specific to Transitions.
