Urram is looking for a supportive, proficient and enthusiastic project officer who will work with our team to develop, launch and run a new support service for West Lochaber, operating throughout the peninsulas.

You will have a concern for the people of West Lochaber and their wellbeing, and an understanding of care and support services.

You will be comfortable and confident in working on your own initiative and in contributing to the wider team.

The role is grant funded through the Support in The Right Direction fund for three years.

Whilst we fully expect the role to grow and develop it is entirely dependent on continued grant support and future relationships with statutory providers.

The Support in the Right Direction project for West Lochaber aims to improve access to social care supports, optimise individuals use of Self Directed Support and enable stronger self-management of long-term conditions.

It will include the development of support hubs in various locations and a West Lochaber personal assistant brokerage network to support connections between local personal assistants and those needing support.

It is part of Urram's work in partnership with In Control Scotland and NHS Highland to develop an alternative model for local coordination and support of social care across West Lochaber.

It is anticipated that there will be a phased approach for launching the service.

Main Tasks

To help Urram develop, launch and run a new service where individuals can come and meet with Urram team members to talk, access supports, be signposted to the right service, connected to existed social and activity groups.

Create an outline for service delivery, utilising existing groups and structures, and establishing new hubs as necessary. As resources allow, ensure the service is delivered in each community council area.
Provide social care and Self-directed Support information through drop-ins, advice points and one to one meetings. Assist individuals with putting outcomes and social care plans in to action (community brokerage).
Publicise and promote the service- including contributing to the regular De Tha Dol article.

Engage with people
Meet with potential and existing individuals using social care services.
Utilise connections with the Community Contacts team for support.
Support individuals and their families to access better supports.
To work effectively as a team member in providing and facilitating a high standard of support in the community.
Signpost individuals to other appropriate agencies, as and when required.

Develop a local network of existing and new personal assistants (PAs) who could be called upon to provide respite or additional support.

Arrange support/training meetings for personal assistants as the opportunity arises.
Support families to become PA employers if required.
Encourage individuals to consider being a personal assistant even if just for a few hours per week.
Build strong relationships with local social workers to support people in our area.

Monitoring
Develop a 'new entry information form' to collect relevant data on service users, along with appropriate filing systems within Office 365 and SharePoint
Hold- as a minimum- bi-monthly reviews with service users to monitor progress against indicators, collecting and collating feedback.
Keep records up to date to ensure all information is captured for service delivery and reporting purposes.
Monitor the demand for the service.

Additional tasks and responsibilities
Work within the policies and procedures of Urram.
Meet regularly with the line manager to ensure smooth running of the project.
Attend staff and board meetings as appropriate.

To identify funding opportunities (with the Board and colleagues) to develop services and so extend the job beyond the current hours and fixed term offered.

Attending training as required.
Any other tasks as they arise to contribute to the smooth running of Urram.
