The post-holder will be expected to undertake all the normal duties and responsibilities associated with a Practice Pharmacist working in General Practice and out of hours setting incl

    Undertake clinical medication reviews with pa
  • tients.Contribute to updating and implementing prescribing
  • policy.Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions, and reviewing medicines reaching review dates, and flagging up those needing a
  • review.Undertake repeat presc
  • ribing.See patients where medicine optimisation is re
  • quired.Incorporate the objectives of medicines optimisation into every day general practice, including the aims of national and local stra
  • tegies.Work with care home staff to improve safety of medicines ordering and administ
  • ration.Reconcile medicines following discharge from hospitals and intermediate care, including identifying and rectifying unexplained changes, and working with patients and community pharmacists to ensure patients receive the medicines the
