Belmont Recruitment are looking for Experienced CPN’s / RMN’s to work an NHS based position, within an older adult CMHT in accordance with the Dementia and Frailty Pathway. The role is located in the Chelmsford area, as part of the Essex Partnership University NHS Trust.
The service is currently looking for experienced CPN’s / RMN’s for a contract position working as a Band 6 Senior Community Nurse. Experience working in the community with the elderly is preferred.
Purpose of the service;
* Provide holistic care to patients in line with the professional code conduct and Trust policies.
* Assess, plan, implement and evaluate packages of care based upon individual service user needs, under the guidance of the Advanced Nurse Practitioner.
Duties will involve;
* Risk assessments.
* Providing documentation on service users.
* Care coordination.
* Provide practical support and guidance.
* Physical health checks.
This position is a full-time working role – working 37.5 hours per week over 5 days (9 to 5).
Pay rates: £25.00 per hour.
