North London Local Authority is seeking a dedicated and experienced Qualified Social Worker to join our Adult Safeguarding Team. This is an exciting opportunity to work in a dynamic environment, ensuring the safety and well-being of adults in our community.
Key Responsibilities:
Conduct individual assessments or support self-assessments using appropriate tools and techniques to establish clear, relevant, and cost-effective options for individuals.
Develop and implement personalised, outcome-focused care and support plans that promote long-term independence and choice, while minimizing ongoing support and lifetime care costs.
Perform risk assessments for adults and carers in line with eligibility criteria and Mental Capacity legislation.
Conduct Mental Capacity Act assessments, ensuring adults have the opportunity to demonstrate capacity wherever possible.
Manage an allocated caseload in compliance with statutory requirements, Council policy, and professional best practice, considering individual choice.
Establish and maintain effective working relationships with local services, specialist teams, ICB and Hospital Trusts, and the voluntary and community sector to deliver holistic and seamless services.
Encourage and value the contributions of adults, their advocates, relatives, carers, and support workers in the assessment, support, and review processes.
Maintain a thorough understanding of available local provisions and make appropriate referrals to other specialist teams and providers.
Promote the use of direct payments where appropriate, provide information, advice, and guidance to support individuals in exercising choice based on assessed risk and expected outcomes.
Collaborate with the Brokerage team to procure care and support, including equipment and adaptations.
Requirements:
Qualified Social Worker, registered with Social Work England
Experience in adult safeguarding and conducting risk and Mental Capacity Act assessments.
Strong understanding of statutory requirements, Council policies, and professional best practices.
Excellent communication and interpersonal skills to liaise effectively with a range of stakeholders.
Ability to manage a varied caseload and work autonomously.
Commitment to promoting independence, choice, and well-being for adults.
