Job Description:
We are seeking an experienced and qualified Adults Social Worker to join our Hospital Team. The successful candidate will be responsible for delivering high-quality social work services, working closely with healthcare professionals, families, and other stakeholders to ensure safe and timely discharges from hospital settings.
Key Responsibilities:
Conduct comprehensive assessments of individuals' needs, risks, and strengths within hospital settings.

Develop and implement tailored care plans, ensuring that all interventions are person-centered and promote independence.

Work collaboratively with multidisciplinary teams to facilitate safe and effective discharge planning.

Provide advocacy and support to adults, ensuring their voices are heard and their rights are upheld.

Manage a complex caseload, maintaining accurate and up-to-date records in accordance with statutory requirements.

Liaise with external agencies, including housing, healthcare, and community services, to coordinate ongoing care and support.

Ensure compliance with all relevant legislation, policies, and procedures, including the Care Act 2014 and safeguarding protocols.
Requirements:
Qualification: Must hold a Social Work Degree.

Registration: Must be registered with Social Work England.

Experience: A minimum of 2 years’ experience within adult social care, preferably within a hospital or similar setting.

Skills: Strong assessment and care planning skills, with the ability to manage complex cases and work effectively under pressure.

Knowledge: In-depth understanding of relevant legislation, policies, and best practices in adult social care.

Communication: Excellent interpersonal and communication skills, with the ability to build positive relationships with service users, families, and professionals.
