Job summary
The post holder will work as part of the Community Intensive Therapy Team (CITT) to support children , young people who present with, complex, high risk, emotional well being and mental health difficulties
They will work with the wider multidisciplinary team to provide support to Children and young people and their family/carer, in the community, during their time of distress.
They will work closely with the team including health colleagues, local authority, Education and 3rd sector.
Main duties of the job
An exciting opportunity has arisen for an enthusiastic and motivated Band 3 Community Health Care Support Worker to join our Community Intensive Therapy Team within the Emotional wellbeing and mental health care group.
The post holder will be expected to support children and young people with moderate to severe mental health difficulties who may face challenges within the community such as tending to their educational and social needs.
The post holder will work closely with the young person and their family or carer to offer community intervention and monitor mental health needs whilst being supported by the wider CAMHS Multi-Disciplinary Team.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome.
