Overview: We are seeking a dedicated Senior Social Work Practitioner for the Preparing for Adulthood Team within the Adult's Integrated Learning Disability Service. The successful candidate will bring a high level of skill and experience in adult social work, particularly within learning disability services, and will play a key role in supervising and managing a team of social workers while maintaining a reduced caseload.
Key Responsibilities:
* Expertise Required: Deep understanding of legislation and policy relevant to adult social care and significant experience in social work within adult learning disability services.
* Supervision and Management: Provide high-quality line management and supervision to social workers in the team.
* Caseload Management: Maintain a reduced personal caseload.
* Transitional Support: Assist adults and young people with learning disabilities transitioning into adulthood, ensuring they receive appropriate support and guidance.
* Multi-Disciplinary Collaboration: Work within a multi-disciplinary team comprising social workers and key NHS professionals, ensuring holistic care and support.
Experience: Supervising social work professionals is essential.
This role is vital in supporting the transition process for young people with learning disabilities, ensuring they achieve positive outcomes through comprehensive and collaborative care
