We are currently seeking a dedicated and qualified Social Worker to join Hackney Adults Services and play a pivotal role in supporting the delivery of health and social care for individuals with long-term conditions.

Role Overview: As a Qualified Social Worker with Sirona Medical, you will be an integral part of Hackney Adults Services. This position is essential in delivering comprehensive health and social care to adults with long-term conditions, including those placed out of the borough. You will lead and manage a team of practitioners, primarily Occupational Therapists, ensuring a person-centred and strengths-based approach to all service users and their families.

Key Responsibilities:

Team Leadership: Lead and manage a team of practitioners, with a focus on Occupational Therapists. Provide line management and supervision to senior practitioners and other team members.

Person-Centred Service: Ensure all staff deliver a person-centred and strengths-based service to service users and their carers/families.

Safeguarding Compliance: Ensure compliance with Safeguarding Adults policies and legislation at both local and national levels.

Performance Management: Manage team performance concerning Health and Social Care Key Performance Indicators (KPIs) and local/team performance measures.

Service Management: Contribute to Freedom of Information (FOI) requests, respond to complaints, and provide necessary information, reports, and feedback.

Service Delivery: Support the delivery of health and social care for people with long-term conditions in Hackney and for those placed out of the borough.

Requirements:

Qualifications: HCPC Registered Occupational Therapist (OT).

Experience: Demonstrated experience in adult social care.

Skills: Strong leadership and management skills, ability to supervise and support a diverse team of practitioners.

Knowledge: In-depth understanding of Safeguarding Adults policies and relevant legislation.

Competencies:

Proven ability to lead and manage a team of health and social care practitioners.

Strong commitment to providing person-centred and strengths-based services.

Excellent understanding of safeguarding adults procedures and legislation.

Effective performance management skills to meet Health and Social Care KPIs.

Ability to handle FOI requests, manage complaints, and provide accurate reports and feedback.
