Here are the details:
Job Title: CPN – Care Coordinator
Band: 6
Location: Greater Manchester
Duration:  3 Months which can be ongoing
Schedule: Full Time 37.5 hours
Rate of pay: £25 to £27 per hour
Your responsibilities will include:
Requirements and qualifications required for this role: 
Current and valid registration with the NMC. 
Proven experience within a CPN recovery role
Strong communication and interpersonal skills. 
Ability to work independently and as part of a multidisciplinary team. 
Commitment to providing high-quality patient care.
Previous experience in working with an Adults team.
To be responsible for the co-ordination, commissioning and personal delivery of individual packages of care for a caseload of service users within CMHTs.
To undertake initial assessments of new referrals to the service.
To support the duty system
