The Role:
As a Targeted Support Worker, you will provide guidance and assistance to vulnerable tenants with complex needs, particularly those with a history of rough sleeping. Your main focus will be to establish and maintain strong personal connections with tenants, enhancing engagement, overcoming obstacles, and preventing tenancy breaches.
You will manage a caseload of up to ten clients, though this number may vary depending on case complexity. These clients will come from various projects and services across the local area, making multi-agency collaboration essential.
Responsibilities Include:
Conduct needs assessments of at-risk and vulnerable tenants.
Provide and coordinate effective support, advice, and assistance to tenants to ensure they can maintain their tenancies.
Support tenants to access support, care, and health services.
Maintain regular contact with tenants, implementing support plans and undertaking appropriate support actions.
Provide case studies and reports on outcomes within the service.Qualifications / Requirements:
A valid UK driving licence and access to own vehicle, with business insurance for roles involving travel.
Experience working with vulnerable adults with complex needs and/or challenging behaviours.
