Cluster Lead

The Dorset ICS are seeking experienced clinical leaders to support with progressing our vision for Home First delivery. These posts, employed by Dorset Healthcare on behalf of the ICS, will be required to work across the system to achieve the strategic ambitions of Home First, and Discharge to Assess.
This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.
These roles are currently fixed term until 31st March 2025 with the intention that this period is used to evaluate and inform the transition to and adoption of a new Transfer of Care Hub model.
To undertake the role successfully there will be a need to utilise clinical experience of out of hospital care, and leadership skills to maximise the opportunity for admission avoidance, flow, assessment following discharge, and successful completion of patient journeys' through discharge pathways using a strength based approach.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check. - Experience of using patient record systems.
- Experience of complex service delivery including multi agency working.
- Able to demonstrate innovation and effective use of resources
- Experience of developing service related plans and strategies and joint working with colleagues in a matrix involvement approach
- Registered health practitioner
- Relevant full Masters degree or demonstrable extensive equivalent experience in the relevant specialty.
- Exceptional proven leadership skills.
- Able to communicate effectively at different levels of the organisation in both written form and in small and large groups together with ability to motivate and persuade individuals and teams, particularly in areas of service and performance improvement requiring a collaborative commitment to change and across organisations, external agencies and all sectors of the community to enable effective relationships
- Ability to monitor and maintain a healthy, safe and secure workplace for self and others
