We have an exciting opportunity to recruit a Physiotherapist.

We are seeking a highly motivated, committed and experienced Physiotherapist to support in the delivery of our service within the North Lancs. You will have the unique opportunity to influence change as we continue to develop physiotherapy within the service and community care.

Our speciality is admission avoidance of which therapy is a key component of the service delivery. Our aim is to support and treat patients in the community, with a range of escalated health and social care needs. Rapid Response's aims are preventing hospital admission and support early discharge from hospital.

The Rapid Response Team is part of the urgent care model and we have an urgent community 2 hour response target. Our service specification includes the provision of Virtual Wards which we have had in place since September 2023. The role out of the Virtual Wards builds on the visions outlined in the NHS Long Term plan and it is exciting to be part of a national mandate. Our virtual ward beds are a mix of Respiratory and Frailty virtual ward beds to provide hospital at home.

Our team is a multidisciplinary team who collectively integrate skills and share knowledge which includes performing non traditional roles and some cross boundary working. In addition to this we work closely with other Community Teams such as the Respiratory service, the UCR Hub, UCAS, District Nurses and many more.

Main duties of the job As a Physiotherapist you will:

Deliver high-quality urgent care within your gift

Have strong clinical decision -making skills

With your expertise and skill, strengthen the care plans delivered from your senior triage and assessments

As part of a multidisciplinary team assess and treat conditions within the patient's own home.

Plan and implement person centre care especially focussing on frailty syndromes and Respiratory conditions, including chest physio.

Help to improve outcomes for patients as well as the patient experience both within the service and wider organisation

Work collectively to improve system working across acute, community and social care settings

Share your knowledge and skill

Utilise best practice

be able to work autonomously

Delegate work

be open and engaging for some cross boundary working ( training and competency assessment will be offered)

Demonstrate an understanding and appreciation for your own role and the different roles within the team and how they work in collaboration.

Undertake timely assessments.

Ensure assessments are of a high standard, with planning, implementation and evaluation of care plans accordingly. About us We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.

FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.

WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.

All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Our community services for adults are provided in people's homes, community centres, clinics, GP Practices, community hospitals and our main hospitals.   • Diploma/ Degree in Occupational Therapy
  • Honours Degree or equivalent in Physiotherapy approved by the Chartered Society of Physiotherapy/ Health and Care Professions Council
  • State registration with Health and Care Professionals Council HCPC
  • Evidence of relevant post graduate specialist clinical education
Desirable
  • Membership of the British Association of Occupational Therapists. or Membership of the Chartered Society of Physiotherapy
  • Post graduate training or experience to post graduate diploma level or equivalent.
  • Accredited Practice Placement Educator's qualification (or willingness to attain within 2 years)
Experience Essential
  • Relevant post registration experience as an OT or physiotherapist to achieve knowledge and skills applicable to this post.
  • Comprehensive rotational experience as a therapist in the core competencies within Occupational Therapy or Physiotherapy for example: musculo-skeletal, neurology and cardio- respiratory care
  • Experience of working within a team
  • Experience as an autonomous practitioner.
Desirable
  • Experience of research or audit.
  • Experience of working as a Therapist of your profession within the community setting such as admission avoidance or rehabilitation
  • Experience of working with frail patients, and knowledge of frailty tools and frailty pathways.
  • Experience of working with patients with Respiratory Conditions.
  • Experience as a supervisor of Basic OT/physio and/or OT/physio support staff and/or OT/physio students
  • Experience of assessment and prescription of aids and minor adaptations.

  • Transferable knowledge and skills at basic practitioner level with aptitude to develop to specialist OT/Physiotherapist level.
  • Ability to analyse professional and ethical issues
  • Assessment, observation, evaluation and reporting skills
  • Knowledge of current best practice in OT/Physiotherapy.
  • Ability to work under pressure and recognise when support is required.
  • Written and oral communication skills in English sufficient to communicate effectively with patients to gain consent and explain treatment interventions and gain information required for the patient's medical/social history relevant to OT/Physio.
