The focus of the role will be to support clients throughout their whole journey with One You...

and this means every step of the way You will be there for all health matters from their very first appointment to their last.

You will conduct healthy lifestyle assessments to identify their needs, and then utilising a behaviour change approach offer support on eating healthier, moving more, drinking less, sleeping better and overall health and wellbeing.

You will also onward refer into our weight management programmes and physical activity sessions.

A typical day as a Health Coach_ _might include:

  • A hybrid model of delivery to support clients both remotely and face-to-face. Around three quarters of your time will be client facing.
  • Conducting healthy lifestyle assessments, which include measurements such as; height and weight, finger prick blood testing, blood pressure and a variety of health and wellbeing questionnaires.
  • Producing tailored plans with the client to ensure they achieve quality outcomes. Provide access to a wide range of basic information and resources, using behaviour change techniques to support sustainable behaviour change.
  • Delivering evidencebased interventions that focus on diet, nutrition, physical activity, behaviour change, and psychological support. Onward refer client's for specialist advice on weight management, smoking cessation and alcohol reduction.
  • Managing a caseload of clients, overseeing their healthy lifestyle journey and engaging with them to maximise their health improvement outcomes.
  • Ensuring client and performance data is recorded accurately and safeguarding incidents are reported and escalated.
  • Conducting measurements using the agreed clinical procedures ensuring that all screening is conducted to the highest standards, in accordance with NHS Best Practice, and NICE guidelines.
  • Working with colleagues across all of our Thrive Tribe services to contribute to new projects and service developments we're always looking to share knowledge and resources
  • Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance.
  • Helping us maintain the ethos of Thrive Tribe by driving our organisational culture forward across our services and using every opportunity to embed our values

Contract:
Permanent, Full-Time (37.5 hours)

Salary:
Starting at
£23,000 per year (rising to £26,000).
