A vacancy has arisen for an enthusiastic and flexible individual to join the Mid Cheshire Hospital Foundation Trust Cardiac Rehabilitation Service.24 hours per week - temporary contract for 12 months.

The MCHFT Cardiac Rehabilitation Team is a hospital based multi-professional team, with community links, to support patients and their families throughout their experience of heart disease.

We offer a menu-based programme to patients who have suffered a recent cardiac event and those who have undergone cardiac procedures, and surgery as well as those with heart failure.

This assistant post provides an excellent opportunity for a new or experienced individual to develop their clinical and professional skills in this new and exciting role


You will be based at Leighton Hospital Crewe and will assist in the delivery of clinics/classes across multi sites in the local community setting.

The role includes clinical administration, organising exercise and education groups, providing support to patients, assisting with the management of waiting lists and communicating with patients and other healthcare professionals

  • To work under supervision to provide an effective clinical support to the Cardiac Rehabilitation team.
  • To be an integral part of delivering the Cardiac Rehabilitation exercise and education programmes
  • To participate under directions, in the assessment of patients within agreed boundaries, and to be aware of and alert a senior member of the team immediately should any adverse reading, symptoms or responses be observed.
  • To support the efficient delivery of services through the provision of administration and coordination of tasks.
  • To assist with the follow up for service users and booking of appointments.
  • To record the patient contact in treatment records, according to local guidelines.
  • To be aware of one's own responsibility to deliver a high quality service at all times.
  • To contribute/enhance existing administration systems to support the organisation and smooth running of the service.
  • To propose and implement new administration processes to assist the team in being more efficient.
  • To contribute to clinical audit processes/administration.
The post holder will work under supervision to provide an effective clinical support to the Cardiac Rehabilitation Team. They will be an integral part of delivering the Cardiac Rehabilitation exercise and education programmes.

The post holder will also support the efficient delivery of services through the provision of administration and co-ordination tasks, assisting with the follow up for service users and booking of appointments together with recording patient contact in patient notes according to local guidelines

  • To assist with the delivery of Cardiac rehabilitation programmes in class settings under the supervision of physiotherapist/exercise instructor and senior nurse.
  • To adjust the exercise within programmes as appropriate within boundaries previously agreed with the Cardiac Rehabilitation team making alterations, in response to varying patient presentation.
  • To involve, encourage and motivate patients to participate in Cardiac Rehabilitation programmes and progress with their lifestyle change plans.
  • To provide, within agreed boundaries education, advice and support for patients and families/carers.
  • To report the senior nurse appropriately on the outcome of your intervention and discuss progress.
  • To input into the monitoring and review of goals.
  • To maintain up to date patient records in written and electronic form in accordance with Trust and local departmental standards.
  • To effectively and efficiently organise and prioritise your own workload
  • To work collaboratively within the Multidisciplinary Team delivering the service to all patients.
  • To assist the team in setting up equipment and the environment in preparation for groups/exercise class sessions.
  • To assist the setting up and running of specialist clinics, including setting out equipment
  • Demonstrate duties to new team members and students
  • To maintain equipment/resources in accordance with manufacturers/departmental guidelines and maintain stock levels.
  • To help keep treatment/office and storage areas tidy.
  • To comply with Manual Handling risk assessment guidelines and action plans.
  • To respect values, cultural and religious diversity of patients and contribute to the provision of a service sensitive to those needs.
