To be responsible and accountable for the organisation and management of a caseload of Home dialysis patients, delivering specialist clinical care appropriate to the needs of the patient.

To perform clinical assessments of patients on a Home Therapy, using up to date knowledge and skills in the planning, implementing and evaluation of care ensuring patients safety at all times.

To promote patients autonomy through education and support to enable individuals to make informed decisions about their care.
To provide specialist training to patients and support person to enable them to safely carryout Home dialysis treatments.

To act in an advisory capacity to provide support and advise to patients, support person,nursing staff and external agencies regarding all aspects of the patients chronic kidney condition.
