Carer Liaison Worker - South Denbighshire


Adferiad Recovery delivers a flexible and coordinated response to the exceptional circumstances faced by people with co-occurring mental health and substance use conditions and related issues.

Vulnerable people facing complex life challenges need consistent and seamless support to ensure they remain engaged with vital health and wellbeing services - and to prevent them becoming disenfranchised and isolated.


Hours:15 hours per week

Salary:
£23,658.53 per annum (pro rata)

Holidays: 25 days per year plus 8 public holidays (pro rata)

Based at:
Denbighshire - location to be confirmed.

Job Purpose:


Under the direction of their line manager, the post holder will undertake support planning, provide one-to-one, group support and interventions to improve the lives of unpaid Carers, families and friends affected by serious mental illness and or Substance misuse issues.


To equip unpaid Carers, families and friends with skills, knowledge, and coping strategies to ensure their own well-being is improved and maintained.

This is to be achieved by visiting clients within their own homes, in the community, at hospital settings and or across groups established across, the South Denbighshire area, thus enabling the unpaid Carers to manage the impact of their caring role.


To work in partnership with other statutory, voluntary, private sector organisations and community facilities to ensure everyone can access other existing services.
