Job Summary
This is an exciting new role within Meridian Primary Care Network Ltd.


In this role you will provide specialist support to young people and their families, with a focus on the neurodevelopmental diagnostic and support pathway.


You will assist with the process of neurodevelopmental assessment, helping to complete the referral paperwork and navigate the local referral routes.

You will guiding the family and young person through this process, making sure they are informed and supported throughout.

Primary responsibilities
The following are the core responsibilities of the PCN Support Time & Recovery Worker (Young People).

There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

  • To work directly with young people and their families, these may be referred to you by staff within the PCN or by direct booking via practice teams.
  • To manage their own caseload.
  • Engage individuals as active partners in their treatment and support.
  • To support families through the assessment process, including signposting and liaising with other parts of the assessment process when required.
  • To provide advice and information to patients and carers, including signposting to other sources of support.
  • To support onward referral to secondary and specialist services where indicated.
  • To develop and maintain close relationships with the surgery teams.
  • Update and maintain patient records.

Secondary responsibilities
In addition to the primary responsibilities, the PCN Support Time & Recovery Worker (Young People) may be requested to:

  • To assertively engage with service users, striving at all times to develop good therapeutic relationships, becoming a trusted person for that family.
  • To develop professional networks, have an understanding of local resources in order to ensure that the service user is able to navigate the full health and social care system.
  • To promote awareness of the professional role across the PCN.
  • To demonstrate effective communication skills with people in an age appropriate way and those whose understanding may be impaired.
  • To act within the appropriate professional code of conduct at all times.
  • To ensure that up to date written and electronic records and activity data are maintained in accordance with Professional standards and to provide specialist reports relevant to practice setting.
  • To keep accurate and timely written and electronic record of each contact, using SystmOne and Emis.
  • To promote and safeguard the welfare of children, young people and vulnerable adults.

Qualifications Essential

  • Good standard of education with excellent literacy and numeracy skills

Experience Essential

  • Working with younger people and their families

Experience Desirable

  • Understanding and experience of working within the NHS
  • Working in the mental health sector

Skills Essential

  • Excellent interpersonal skills, communicating with patients, their families and healthcare professionals
  • Ability to maintain accurate records
  • Be flexible and able to manage sudden and unexpected demands
  • Effective time management (planning and organising)
  • Demonstrate personal accountability, emotional resilience and work well under pressure
