Job Description:

Key Responsibilities:

  • Assessing the needs of adults who are being discharged from the hospital, including identifying any social, emotional, or practical support that may be required
  • Working with patients and their families to develop care plans that meet their needs and support their independence
  • Coordinating with other professionals and community resources to ensure that patients receive the necessary support and services following discharge
  • Providing counseling and support to patients and their families as needed
  • Maintaining accurate and uptodate records of all patient interactions and care plans
  • Participating in team meetings and case reviews to ensure the best possible outcomes for patients

Requirements:

  • Bachelor's degree in social work or a related field
  • Valid social work license in the UK
  • At least 2 years of experience working in hospital discharge or a related field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Proficiency in Microsoft Office and other relevant software
