Job summary

This is a Social Worker post which will improve outcomes for adults in Buckinghamshire. There may be opportunities to work with older adults and our transitions team, depending on needs of service user group. You will be supported by an Assistant Team Manager and work within a small team who have close links with the Buckinghamshire Council as well as working alongside community health colleagues within Oxford Health NHS Foundation Trust. You will use relationship based Social Work skills to understand the needs of this adult service user group and work collaboratively to create care and support plans that reflect person-centred outcomes.

Our social work team within community mental health has arisen out of a renewed commitment by Oxford Health NHS Foundation Trust and Buckinghamshire Council to continue working in partnership to deliver mental health services for the residents of Buckinghamshire and those placed out of area for whom we are the responsible body.

The post-holder will have strong Social Work values and a commitment to improving the lives of adults with complex health and social care needs.

Main duties of the job

Be an ambassador and leader across social work both internally and external to the department.

Work with adults, and their families, who may be placed in a range of community and placement settings, to form an understanding of their needs and wishes and develop strength-based plans.

Understand and apply the relevant statutory frameworks to promote independence and autonomy while balancing risks and the needs of others including children and vulnerable adults.

To provide a high quality and effective professional casework service, taking responsibility for working and managing a caseload within a framework of appropriate supervision, dependent upon the complexities of these cases and the social worker's experience.- To comply with legislation, statutory guidance and local policies and procedure.- Delivering outstanding support to Buckinghamshire residents and ensure that regular monitoring and review of care and support plans is carried out.- To be competent in all areas of the Professional Capabilities Framework of Social Work.- Demonstrate and model good practice that reflects a commitment to relationship and strengths-based practice, working collaboratively with adults and their families.

Person Specification

Skills and Experience

Essential

Experience of working with working as a registered social worker in a statutory setting More than one year continuous post-qualifying practice. Experience of working within relevant Legal frameworks. Experience of working with families and carers.

Desirable

Experience of working in Adults in a mental health setting AMHP or BIA qualification PEPS 1 or 2 award undertaking assessments, reviews and s42's for adults under The Care Act 2014
