As a Health Visitor you will individually and within a team be accountable for the management and delivery of programmes of care for a designated caseload/population.

Ensure the delivery of clinically effective practice through the effective utilisation and integration of evidence based practice and implementation of evidence based standards of care, policies, procedures and protocols.

Ensure a culture of continuous quality improvement through the use of audit, patient/family/stakeholder feedback and reflection on practice by self.

Ensure the identification of the health needs of families and communities, using appropriate methods and tools.


Promote a clean and safe environment for staff, children and families by ensuring compliance with legislation, policies and protocols including health and safety, healthcare associated infection prevention, clinical and information governance, including risk management, critical incident reporting and root cause analysis.

Ensure effective communication with the multi-professional team, statutory voluntary organisations and other partners to ensure all care given is appropriate to the child and family and to prevent duplication of services.

Ensure a high standard of care and record keeping in accordance with Nursing and Midwifery Council, national legislation and local standards.

Report incidents and near misses promptly and appropriately and take effective action to minimise future risk.


Ensure an effective and efficient child and familys journey by planning and co-ordinating the episode of care including the smooth transition to other settings, promoting effective communication with interdisciplinary and interagency teams as required.

Identify vulnerable families and children at risk and participate in safeguarding activities, looked after children reviews, child in need and CAFs as appropriate to promote positive health and prevent deterioration of the child and familys health and wellbeing.

Assess and interpret clients conditions, deliver and evaluate health visiting interventions.

Influence clinical decisions and monitor the quality of child and family care provided through clinical knowledge relevant to own field of practice, underpinned by theory and experience.

Within a multidisciplinary team environment, develop and embed a culture of family-centred care placing the child at the centre.

Be highly visible within the service, communicating regularly with children and families and/or carers; promote a caring environment where equality and diversity issues are respected and families are enabled to be partners in their care.

Identify opportunities to develop care and services for the client groups.

Ensure that everyone within the sphere of care is treated with dignity and humanity, understanding individual needs, showing compassion and sensitivity, and provide care in a way that respects all people equally.


Maximise all opportunities to contribute to the health education/ promotion agenda and principles of every contact counts Identify local public health needs by analysing and utilising public health data and work with the community to build community capacity Act as a visible role model, creating a supportive ethos to empower staff to contribute to the delivery of high quality person centred care.

Lead by example, develop yourself and other staff, and influence the way care is given in a manner that is innovative, open and responds to individual needs.

Participate in the appraisal process identifying personal development needs that are consistent with the team, service and organisations objectives.

Utilise robust communication networks within and external to the team Take all reasonable care of health and safety at work for you and others and to co-operate with employers to ensure compliance with health and safety requirements.


Demonstrate efficient use of time management to meet competing demands of the service Support the learning and development of all staff.

Support the learning environment ensuring effective learning opportunities for staff and students including appropriate orientation and induction programmes, a range of clinical support strategies (mentoring, coaching, clinical supervision and action learning).

Plan own mandatory training and relevant education.

Act as a mentor to students on healthcare programmes Ensure compliance by self and others with professional standards, legislation, national and organisational policies.


Support local recruitment and selection, attendance management, and performance management ensuring grievance and disciplinary matters within own practice setting are identified, actioned and reported to the appropriate person Ensure cause for concern issues are managed and escalated appropriately Manage a designated caseload, through which you provide universal preventative healthcare services to young children and families.
