RT Healthcare are looking for an experienced Locum Care Manager to work closely with Health colleagues to facilitate a safe return home for Vulnerable adults who present in the Emergency Department at the Princess Royal University Hospital in Farnborough,Bromley.


  • 3 month's contract_

Duties include:

  • You will be required to complete assessments under the Care Act 2014 and Mental Capacity Act 200 arrange care support to enable adults to return home to reduce the risk of social admissions to hospital or temporary placements to meet initial needs until further assessments can take place.
- attend board meeting to discuss patient flow and plan the discharge.

  • Once the adult has returned home the case will be followed up by the respective Locality teams.
  • You will have a knowledge/experience of facilitating safe Hospital discharges, strong working knowledge of the Care Act 201
  • Timely strengthbased assessments Also, knowledge of the Mental Capacity Act and experience completing Mental Capacity assessments.
- you will be supporting Younger Adults with physical disabilities and sensory impairment and Older People to live as independently as they are able and work with their carers to ensure risks are appropriately managed.

You will need to be flexible, committed, and hard working with a willingness to work to quality standards.

You may also be required to work weekends.
