Participate in the planning and evaluation of the individual care programme with Community Psychiatric Nurse. Promote and support patients/clients rights, beliefs and choices taking into account issues of diversity and cultural needs. Participate in ensuring that patients/clients are protected from any form of abuse. Establish and maintain professional relationships with patients/clients, relatives, friends and other members of staff.


Support patients/clients to maintain their interests, identity and emotional well-being whilst receiving various forms of care, observing and reporting any changes in their condition, finance, environment etc.

Encourage patients/clients to maintain their personal hygiene and appearance. To actively encourage social inclusion for service users. Encourage patients/clients to eat and drink whilst following a balanced and nutritious diet. Assist patients/clients to manage their personal and domestic circumstances.

Accompany service users to appointment i.e. outpatient clinics. To write clear concise and legible entries in service users notes after each visit. To attend team meetings.

To participate in supervision as agreed with the Team Manager. Any other duties commensurate with the grading of the post. Undertake relief duties as appropriate for the maintenance of the service. Be familiar with Pennine Care NHS Trusts Policies and Procedures.
