Job Title: Support Coordinator

Creative Support is a non-profit provider of personalised care and support to individuals with diverse needs. We are seeking a Support Coordinator to join our team in Ashton-Under-Lyne, Tameside.

About the Role

As a Support Coordinator, you will be responsible for developing and implementing creative activities for individuals accessing our service. You will work closely with service users, families, and the multi-disciplinary team to maximise service user outcomes.

Key Responsibilities
  • Develop and implement a monthly timetable of creative activities for individuals accessing our service.
  • Provide respectful and bespoke personal care, emotional, and practical support to each person.
  • Collaborate with service users, families, and the multi-disciplinary team to maximise service user outcomes.
  • Implement Creative Support principles, policies, procedures, and guidelines, ensuring the service meets CQC requirements.
  • Supervise and support staff to best meet service user needs and preferences.
Requirements

Experience of working with older people and/or people with dementia is essential. You must be at least 21 years old for this role. Possession of a driving license is essential for this role, as you will be expected to drive a 12-seater mini-bus.
