About the Role

Creative Support is a leading provider of personalized care and support services. We are seeking a highly skilled and compassionate Support Coordinator to join our team.

Key Responsibilities
  • Develop and implement community outreach programs to extend our services to a wider audience.
  • Design and deliver a monthly schedule of creative activities to promote participation and engagement among service users.
  • Provide respectful and personalized care, emotional, and practical support to each individual.
  • Collaborate with service users, families, and the multidisciplinary team to maximize service user outcomes.
  • Implement Creative Support principles, policies, procedures, and guidelines to ensure compliance with CQC requirements.
  • Supervise and support staff to meet service user needs and preferences.
Requirements
  • Experience working with older people and/or individuals with dementia.
  • Ability to work on a rota, including weekends and bank holidays.
  • Valid driving license and ability to drive a 12-seater mini-bus.
