We are looking for Band 6 Case managers to support the new Keyworking Service for children and young people (CYP) aged 0-25 years, who have learning disabilities and / or are autistic. The service will work with those who have the most complex needs and are either at risk of admission or at inpatient.

We are looking for passionate and motivated professionals with extensive experience working with children and young people's services (0-25 years) who can become experts in the range of services across the North East London boroughs. They must be able to build strong partnerships with other NELFT services and NEL Partners and external stakeholders to ensure service delivery is of high quality whilst promoting NELFT values.

Candidates must have direct experience of working with families with children or adults who have complex needs with an ability to support families and young people to navigate through care systems and pathways. They must have an ability to empower parents/carers and individuals and to work in a co-productive style.

Main duties of the job

The post holder will be responsible for a caseload of children and young people aged 0-25 years old who have been referred to the Keyworking Service from Dynamic Support Registers across North East London. The post holder will provide care coordination and act as the key worker across health, social care, education and voluntary sectors to manage the care and support of children and young people to prevent admission to hospital, or reduce length of stay in hospital if the young person is already an inpatient.

The post holder will ensure that there is a coordinated multi-agency plan in place to support patients on their caseloads. Patients are at the heart of the model, and in partnership with professionals, supported by dedicated case management. All involved, including the person and the carer, will design care plans together to keep the person well for as long as possible and achieve outcomes they identify.

The post holder will be responsible for ensuring that the service meets the agreed key performance indicators (KPIs) and is operating in line with the Care Quality Commission (CQC) guidance and that the service is effectively monitored and delivered in a cost-effective manner whilst ensuring that the care and support given is of the highest standard.

The post holder will be responsible for ensuring appropriate information is readily available to commissioner's, senior manager, CQC and local authority as required and that this information is maintained and up to date.

About us

Starting with NELFT NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff). High Cost Area Supplement - London This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum).

Qualifications

Essential

Educated to degree level or equivalent experience.

Desirable

Registered Nurse (RMN Mental Health, LD, RGN) or AHP or Dip SW, CSS or CQSW.

Experience

Extensive experience of working directly with young people with complex, acute mental health conditions and challenging behaviours who may be autistic and/or have learning disabilities to develop personalised support plans or equivalent

Knowledge

Essential

Understanding of care planning, coordination and risk assessment and management within community settings.

Desirable

Understanding of the background to and aims of current healthcare policy in London and nationally on DSR, C(E)TRs and keyworking. Knowledge of the Mental Health Act, Mental Capacity Act, and related legislation, codes of practice and guidance.
