- Provide direct care and support to individuals in need, including seniors, patients, and those with disabilities
- Assist with activities of daily living, such as bathing, dressing, grooming, and meal preparation
- Administer medications and monitor vital signs as directed by healthcare professionals
- Collaborate with case managers and nurses to develop and implement care plans
- Assist with mobility and transfers, ensuring the safety and wellbeing of individuals
- Provide emotional support and companionship to individuals
- Document observations and report any changes in condition to the appropriate healthcare professionals
- Previous experience in a similar role preferred
- Knowledge of medical terminology and experience in hospice care is a plus
- Familiarity with patient care techniques and best practices
- Ability to provide compassionate care to individuals with diverse needs
- Experience in behavior management is beneficial
- Understanding of case management principles and practices
Skills:
- Strong communication skills, both verbal and written
- Excellent interpersonal skills, with the ability to build rapport with individuals and their families
- Ability to work effectively as part of a team, collaborating with healthcare professionals and other support workers
- Attention to detail and the ability to follow instructions accurately
- Strong organizational skills, with the ability to prioritize tasks effectively
