JOB SUMMARY:

  • To be part of a core team responsible for leading the development and delivery of a high-quality Acute Service for adults admitted to the stroke service.
  • To contribute to defining a delivery model that ensures a seamless and efficient pathway for service users.
  • To take on a leadership role in planning, coordinating, developing, and evaluating clinical services by utilizing advanced clinical skills and knowledge.
  • To assess, plan, and provide care for patients in the Neuro/ASU wards, including participation in the weekend SLT rota.
  • To assist in ensuring the safe discharge of patients by collaborating with existing acute and community services and staff as necessary.
  • To handle a diverse caseload, applying evidence-based and person-centered approaches to assess, plan, implement, and assess interventions.
