Generic Support Worker

Supporting adults with a learning disability, must be manual handling trained, deliver personal care, supporting to access activities in the community.


RESPONSIBLE TO:

SENIOR COMMUNITY SUPPORT WORKER

JOB DECRIPTION


Function as part of a care team which enables service users to maximise their social, emotional, and physical independence to the absolute maximum, in line with the individual service user's care plan and the principles of recovery/re-ablement.

Ensuringthat individual plans, which are outcome focused, are implemented and reviewed in a timely way with any changes being communicated to the relevant people.


PRINCIPAL RESPONSIBILITIES

Deliver a range of services to service users to ensure their independence is maximised in line with the outcomes set in the individual plans, encouraging and enabling service users where possible to undertake tasks for themselves, eg dressing, washing, feeding,socializing, managing finances.

Administering non-invasive medication where necessary and in line with the care plan.

Carry out assessment tasks, maintaining and updating records as required, and undertake regular checks and report any significant issue to the relevant person and your supervisor, where necessary take immediate action.

Maintain dignity for service users and encourage users to maintain control and choice in their lives and ensure that service users are at the centre of and they make use of integrated services to maximise their independence.

Contribute to discussions on Individual Care Programmes for service users, act as a key worker, and ensure the proper completion of both client and management records in accordance with service policies.

In the event of situations likely to threaten the safety or security of service users implement any initial action within defined procedures and report immediately to the Senior Support Worker (SSW).

Undertake simple domestic tasks and additional duties as identified by the SSW with due attention to Health and Safety legislation and appropriate hygiene standards.

Liaise with service users and help service users to maintain contact with family, friends and community in a professional manner, whilst respecting confidentiality at all times.

Participate in the delivery of social activities as identified on care programmes, or as directed by the SSW.

Maintain a full and up to date awareness of relevant internal policies and procedures and participate in training activities to ensure that knowledge and skills remain relevant and upto date, meeting service requirements.

Shift patterns are 8-3 and 3-10 individuals may works 8-10 if preferred consistency of staff preferred

Must be manual handling trained
