Patient Transport Coordinator

The Patient Transport Services department continues to establish itself as a single point of contact for ordering all patient transport across Cornwall and the Isles of Scilly.
Working in a busy team, the successful applicant will be required to receive, assess and co-ordinate requests for patient transport across the Trust with the transport available.
You will be required to demonstrate the ability to communicate at all levels and function efficiently within a pressurised environment.
Receive, assess, co-ordinate and advise on requests for Patient Transport. This may involve calls from individual patients / relatives / carers in respect of their transport arrangements.
Where patients meet eligibility criteria, organise the appropriate transport to transfer patients to Community Hospitals, Intermediate Care Homes and other NHS health facilities, as well as private addresses, in liaison with TAP (Transport Access for Patients), SWAST (South West Ambulance Trust) and Private Ambulance companies.
To maintain database records of all transport requests
